In plain English
The biggest thing Barque missed this month: back in November, the US government quietly removed its strongest safety warnings from hormone replacement therapy — the medications millions of women use for menopause symptoms. Since then, prescriptions have roughly doubled. Demand for estrogen patches has surged 184% since 2023. The country is now running short, with one major patch brand in nationwide shortage and supply potentially tight for years.
This pattern looks familiar. Two years ago, a regulatory shift on weight-loss drugs like Ozempic unlocked pent-up demand, overwhelmed supply, and opened a secondary market at specialty pharmacies. The same template is now playing out in hormone therapy.

How Barque got smarter today
- 22-day gap on HRT black box removal cascade. Bloomberg published "Hormone Therapy Prescriptions Jump as FDA Removes Black Box Warnings" on April 8. The FDA labeling changes were approved in February. Ra has been running daily since April 19 and never searched for menopause/HRT signals until today. Root cause: search queries focused exclusively on peptides, GLP-1, and pet health. Zero of the prior 11 Ra runs included a menopause-domain search. Corrective: add "menopause HRT 2026" to every run's search rotation. Menopause is Ring 1 and must be sampled.
- Coverage gap: menopause-queenager has zero pending forecasts. Of five Ring 1 domains (GLP-1/metabolic, peptides/longevity, skincare, menopause/HRT, pet health), menopause has been tracked zero times in forecasts.tsv. This is a structural blind spot, not a quiet domain — the demand signal is loud. The proposed estrogen-patch-shortage forecast begins to fill the gap.
- Cross-domain template recognition. The HRT shortage following the GLP-1 shortage template (Case 2 in historical-cases.md) is a generalizable heuristic: when FDA removes a regulatory barrier on a supply-constrained therapeutic class, expect demand to outpace supply for 12–18 months. Promoting to heuristic candidate for evolution log review.
